Send Off Report Form

  • IMPORTANT MESSAGE ABOUT FAILED SUBMISSIONS

    Referees who, after clicking the "Submit Send Off Report Form" button, see a blank report form have not successfully submitted the report form.

    Referees who have successfully submitted the report form will see the following message:

    You have successfully submitted a send off report form.

    Your submission will be reviewed by the MWFRA prior to being forwarded to the MWFA Judiciary.

    If your report requires amendment, you will be shortly contacted to revise and re-submit your report.

    Referees should save their typed up report elsewhere before submitting to avoid any potential technical issues that may result in loss in progress.

    Referees who encounter an error in submitting the report form should immediately email [email protected] and [email protected] with details of the send off.

  • EXAMPLE

    In the 50th minute, I sent off Team A #10 for serious foul play for an over the ball tackle which endangered the safety of an opponent.

    Immediately prior to the incident, a player from Team B was running with possession of the ball on the halfway line near the touchline.

    Team A #10 ran towards the ball from a distance of five metres and subsequently jumped towards and over the ball with two feet extended together and locked at the knee into the player from Team B.

    Team A #10 jumped in with moderate speed and his studs on both boots made low contact with the player from Team B approximately halfway up the shin. The contact was not flush but rather more a graze.

    I was located approximately 10 metres away towards the centre circle and had a clear and unobstructed view of the challenge. I stopped play and dismissed Team A #10 from the field of play. The player left the field of play without incident.

    The player from Team B did not require treatment and was able to continue. Play was restarted with a direct free kick to Team B.

  • EXAMPLE

    In the 50th minute, I sent off Team A #10 for violent conduct for punching an opponent.

    Immediately prior to the incident, Team A #10 had possession of the ball just inside the centre circle. Team B #9 challenged him for the ball, which resulted in both players falling to the ground. I deemed the challenge to be fair.

    Immediately following the challenge, Team A #10 got to his feet and walked directly towards Team B #9 and then punched Team B #9 using his right hand, which was in a tightly clenched fist. The punch was of moderate force and made contact with the left chin of Team B #9.

    Following the punch, players from both teams ran towards the incident and separated the two players.

    I was located approximately 5 metres away and had a clear and unobstructed view of the punch.

    When players from both teams separated, I dismissed Team A #10 from the field of play. The player left the field of play without incident.

    The player from Team B did not require treatment and was able to continue. Play was restarted with a direct free kick to Team B.

  • EXAMPLE

    In the 50th minute, I sent off Team A #10 for spitting at an opponent.

    Immediately prior to the incident, Team A #10 had possession of the ball just inside the centre circle. Team B #9 challenged him for the ball, which resulted in both players falling to the ground. I deemed the challenge to be fair.

    Immediately following the challenge, Team A #10 got to his feet and walked directly towards Team B #9. While standing over Team B #9, who was sitting on the ground at the time, Team A #10 then spat at him.

    Team B #9 raised his left arm over his head to deflect the spit, which resulted in the spit landing on his left forearm.

    Team B #9 immediately got up and confronted Team A #10 and players from both teams ran towards the incident and separated the two players.

    I was located approximately 5 metres away and had a clear and unobstructed view of the spitting and where it landed.

    When players from both teams separated, I dismissed Team A #10 from the field of play. The player left the field of play without incident.

    Play was restarted with a direct free kick to Team B.

  • EXAMPLE

    In the 50th minute, I sent off Team A #10 for denying the opposing team a goal by deliberating handling the ball.

    Immediately prior to the incident, Team B was awarded a corner kick. The ball was kicked towards the penalty spot and a Team B player headed the ball in the direction of goal near the left post.

    Team A #10 was standing on the goal line next to the left post and, following the header, subsequently moved his left arm about 30 centimetres away from his body to block the ball from entering the goal.

    The ball struck the player's left forearm and rebounded away from the goal. Had the ball not been blocked, the ball would have entered the goal.

    I was located approximately 10 metres away towards the left side of the penalty area and had a clear and unobstructed view of the incident.

    I immediately dismissed Team A #10 from the field of play. The player left the field of play without incident.

    Play was restarted with a penalty kick to Team B.

  • EXAMPLE

    In the 50th minute, I sent off Team A #10 for denying the opposing team an obvious goal scoring opportunity by an offence punishable by a free kick.

    Immediately prior to the incident, Team B had launched a counter attack form their own half. Team B had lobbed the ball past the Team A defensive line. A Team B striker gained possession of the ball halfway between the halfway line and the goal with no other defenders except the goalkeeper between the player and the goal.

    Team A #10 gave chase to the Team B striker and, as the Team B striker entered the top of the penalty area with the ball and was about the shoot, Team A #10 pushed the striker over with both hands. The Team B striker fell over.

    I was located approximately 15 metres away towards the left side of the penalty area and had a clear and unobstructed view of the push.

    Because of the attacker's close distance to the goal, the attacker's control of the ball, the attacker's direction of movement towards the goal, and the lack of nearby defenders to fairly intervene in the situation, in my opinion, the push unfairly denied the attacker an obvious goal scoring opportunity.

    I immediately dismissed Team A #10 from the field of play. The player left the field of play without incident.

    Play was restarted with a penalty kick to Team B.

  • EXAMPLE

    In the 50th minute, I sent off Team A #10 for using offensive language towards me.

    Immediately prior to the incident, Team A #10 had possession of the ball just inside the centre circle. Team B #9 challenged him for the ball, which resulted in both players falling to the ground. I deemed the challenge to be fair.

    Immediately following the challenge, Team A #10 got to his feet and looked directly towards me and shouted "What was that ref?" I replied that it was a fair challenge. The player than shouted "Why don't you put on a blue shirt [the colour of team B] you cheat?"

    I was located approximately 5 metres away from the player at the time of the incident.

    I immediately stopped play and dismissed Team A #10 from the field of play. The player left the field of play without incident.

    Play was restarted with an indirect free kick to Team B.

  • No description is required for R7 send offs.

  • The description must, at a minimum, comprise of a detailed description of:

    • the lead up to the offending action;
    • the offending action itself;
    • the force, nature and location of the contact;
    • the aftermath of the offending action;
    • the players involved;
    • the location of the offence;
    • the location of the ball;
    • the location, closeness and view of the referee;
    • the response of the sent off player; and
    • the method of restarting play.

    Extended dot point responses under each heading is acceptable.
